Fees & Policy Questions
Fees:
Dr. McLemore is an out-of-network provider with all insurance payors. All fees are due at the time of service. You may submit claims to your insurance company following appointments. I will provide superbills to you upon request.
If you are unsure if your insurance will cover out-of-network care, call them and ask them about your out of network benefits.
$280—Intake Appointment (insurance code 90791)
$280—60 minute Individual Therapy (insurance code 90837)
$210—45 minute Individual Therapy (insurance code 90834)
$140—30 Minute Individual Therapy (insurance code 90832)
$50—Letter writing
$70 per 15 minutes of consultation
$150—Missed appointment fee
What is the missed appointment policy?
If you miss an appointment or cancel in less than 24 hours, a missed appointment fee will be charged to you unless you are able to reschedule the appointment within the same week. This is subject to the therapist having availability to reschedule.
Are there exceptions to the policy?
Exceptions to the missed appointment policy are only made in the event of true emergencies. Documentation of the emergency may be requested.
Do you take insurance?
If you have TriCare, CTC has a special rate for you as a certified non-network non-preferred provider. This means your TriCare Insurance will reimburse you for the majority of the cost of our session. More details can be provided for you during our phone consultation.
Do you offer a sliding scale?
CTC does offer a sliding scale fee to patients who qualify. If you think finances are going to get in the way of your treatment, let’s talk about it and see if you’re a good fit for a sliding scale.
Are after school appointments available?
After school appointments are not available at this time. CTC is willing to look at the school calendar with you to see if we can make appointments on school holidays. CTC will also provide you excused school notes upon request.
